A completed application for the MS or PhD degree consists of the
following:

Graduate School application (completed online)

School of Public Health Supplemental Application
(PDF)
- Transcripts from all school attended
- Scores from the Graduate Record Exam; International
students must also submit TOEFL exam scores
- WES Verification for transcripts of International
schools
- Three letters of recommendation
- The applicant's personal statement of intentions, and
- The applicant's resume
